To get the best ending in The Forgotten City players must make sure they complete these four essential steps:

  • Gather all four plaques and insert them in the obelisk outside The Great Temple.
  • Save Iulia.
  • Save Fabia.
  • Save Ulpius.

Once Iulia, Fabia, and Ulpius have been saved once and Malleolus' real name has been exposed for the first time, players can get Galerius to save them and expose Malleolus instead when talking to him at the start of a new loop.
