The Rold Medallion is given to players by Melina after they defeat Morgott, The Omen King in Leyndell, Royal Capital. Upon beating the Omen King, no Site of Lost Grace appears like how it usually does after killing Elden Ring bosses. That's because Tarnished must first climb the grand staircase ahead and interact with the locked entrance of the Erdtree. After that, they can go down the stairs and the bonfire should be there.

The moment players rest at the Site of Lost Grace, Melina, the Maiden, appears and tells them that it's currently impossible to enter the Erdtree and face what's beyond it. The only way is to travel to the highest point in Mountaintops of the Giants and burn the Erdtree. Melina then gives players the Rold Medallion and leaves a red mark on the map.

Once here, they should light the bonfire, then summon Torrent and ride northeast. Players will know they're in the right place when the Black Blade Kindred boss appears. Instead of fighting it, they should run straight past it into the building and light the Site of Lost Grace to the right. Next, they'll need to climb the stairs and use the Elden Ring medallion to activate the Grand Lift of Rold. The lift will then take Tarnished to the Mountaintops of the Giants.
